What to Bring

Please bring the following to your appointment:

• The referral from your GP, if not already faxed to the rooms.

• Your list of medications.

• Relevant X-rays, CT scans and blood tests.

• Your Medicare card and credit/bank card (can be useful for online Medicare reimbursement).
